PERPP Recordkeeping: A Review of the Process

As 2025 concludes and we begin a new year, please be reminded that your PERRP Recordkeeping logs, summaries, and online reports are due by February 1st, if applicable. While the recordkeeping process can be complex, the following information can help provide clarity. 

  1. All school districts are required to maintain a PERRP log for each facility. Facilities include high schools, middle schools, elementary schools, transportation buildings, and administrative offices.
  2. Download the recordkeeping forms and instructions and accurately record all injuries occurring during the calendar year that meet the criteria for being "recordable." Please note that not all injury claims will be considered recordable.
  3. Submit your annual injury summary information by Feb. 1, unless your organization qualifies for an exemption.

Let’s take a look at each step in more detail.

Exemptions

There are no exemptions for keeping and maintaining the PERRP Log.  All school districts are required to keep the log.

Recordkeeping Logs and Summary Forms 

  • Download the recordkeeping instructions here.
  • You can find the PERRP log forms here. Along the right side of the page, you may download and complete PERRP Microsoft Excel recordkeeping forms (XMLS) and Recordkeeping forms (300P, 300AP, 301P) and instructions (PDF).
  • When your PERRP log is complete for the 2025 calendar year, you will need to complete the PERRP 300AP Summary form and have it signed and posted in a conspicuous place for employees to see.  Summaries must be posted from February 1st through April 30th.

Remember to post only the PERRP 300AP Summary and not the injury log. 

Online Submission

The final step in the process is to determine if you must file online.   

All school districts are required to submit to the BWC, except those with five or fewer employees and those that have had no recordable injuries in the calendar year.  PERRP 300AP Summary form information is submitted online through the Ohio BWC link here.

Remember, even though you must complete a log for each establishment, school districts are only required to submit one master 300AP that includes totals from all establishments.
